Hospital or health department based Diabetes Self-Management Education programs across the state are certified by the Michigan Department of Health and Human Services (MDHHS) for Medicaid purposes. The certification only comes when the program shows it meets quality requirements. Programs can accept Medicaid when MDHHS certified (but they take other insurances, too).
We have two ways to find a program. Move your mouse over a city or area and a pop-up will appear with the local hospital
name, city, and a phone number. Click on the map and the location information continues to display.
Or, view a list
of all the programs in the
state that are MDHHS certified. They are listed by hospital name.
